Who is Eligible?
Employee
Active, full-time employees who are expected to regularly work 30 or more hours per week become eligible for benefits on the first day of full-time employment.
Dependents
Eligible Dependents include:
- The employee’s legal Spouse (as defined in the carrier plan document).
- An employee’s Child up to their 26th birthday (as defined in the carrier plan document).
A “Child” is (Proof may be required):
- An employee’s natural born child or legally adopted child.
- An employee’s disabled child over the age of 26.
- An employee’s stepchild
- An employee’s grandchild who qualifies as an exemption under the Internal Revenue Code.
- Any child for whom the Employee has been granted legal guardianship by a court of law.
- Any child for whom the employee is required to provide health care coverage under a Qualified Medical Child Support Order.
If an Employee does not enroll him/her self and eligible dependents within 31 days of the date eligible, they will not be eligible to enroll in the Plan until the next Open Enrollment or through a qualified Special Enrollment.
